All independent contractors, staff, and any other personnel involved in the presentation
and during my participation in school classes.

All Personal Continuing Education Classes are accepted on a first come first serve basis.
For your protection … proof if current state license and/or current student registration/ID Card will be required along with a
photo ID. Unless noted that supplies will be provided please make sure to bring your own equipment, supplies and
implements. I understand that I will be working and attending at my own risk and will forever hold harmless said A.B.C.
SCHOOL OF COSMETOLOGY, ESTHETICS & NAIL TECHNOLOGY, INC all independent contractors, staff, and any other
personnel involved in the presentation and during my participation in any of the above named courses. If the class is a
“hands on class” * models are allowed and will participate solely at their own risk. Models must be a minimum of 16
years or older and a signed permission slip for services rendered if under age 18 years of age by a legal guardian and/or
parent. Minor release forms available at time of class or by contacting School Office.

IF YOU CAN NOT MAKE ANY OF THE CEU CLASSES FOR 2011? THEN PLEASE FILL OUT THE
BELOW FORM OR CALL CINDY HEIDEMANN AT (847) 458-6500. WE WILL CALL YOU BACK TO
SETUP AN HOURLY SCHEDULE IN OUR ACADEMY. HOURLY FEES ARE SUBJECT TO CHANGE
AND AVAILABILITY.
