| CEU CLASSES FOR 2008 |
| If you need Continuing Education Hours and if you are a Nail Technician, Nail Instructor or Cosmetologist. Remember 2008 is a License Renewal Year for Nail Technicians. |
All registrations are accepted on a first come first serve basis. Registration fees will be refunded at 25% prior to the cut
off date for each individual class. No refunds will be issued after the cut off date.
For your protection … proof if current state license and/or current student registration/ID Card will be required along with a
photo ID. Unless noted that supplies will be provided please make sure to bring your own equipment, supplies and
implements. I understand that I will be working and attending at my own risk and will forever hold harmless said A.B.C.
SCHOOL OF COSMETOLOGY & NAIL TECHNOLOGY, INC. – SUGAR & SPICE PARTIES & SPA, INC. all independent
contractors, staff, and any other personnel involved in the presentation and during my participation in any of the above
named courses. This is a “hands on class” * models are allowed and will participate solely at their own risk. Models
must be a minimum of 16 years or older and a signed permission slip for services rendered if under age 18 years of age
by a legal guardian and/or parent. Minor release forms available at time of class or by contacting School Office.
